《史记·扁鹊仓公列传》载：虢太子死，扁鹊过虢，自荐能生之，虢中庶子好方技者不信，扁鹊“仰天歎曰：‘夫子之爲方也，若以管窺天，以郄視文。’”郄（隙）即穴。后因以“管穴”喻狭隘的识见。
如其管穴，妄有譏刺，雖苦口逆耳，不得事實，且優遊寬容，以示聖朝無諱之美。
